Connecticut Health Requirements and Forms
Connecticut law states that no one may attend school without the medical documents identified below, unless written exemption is given on medical or religious grounds.
The Health Assessment form (HAR-3) is mandatory for Pre-School, Kindergarten, grade 3, grade 6, grade 9, and CHS Athletics physicals and MUST have the doctor's signature AND stamp.
Required Physical Examinations:
ALL new students (including kindergarteners) must show proof of having received both a series of required immunizations and a recent (within 12 months) physical examination by a physician.
Kindergarten, Grade 3, Grade 6 and Grade 9 Students:
ALL Kindergarten, grade 3, grade 6 and grade 9 students are required to provide proof of a physical examination completed during the school year. Immunization update required.
- To claim an immunization exemption based on religious beliefs, a child’s parent or guardian must present a statement that such immunization would be contrary to the religious beliefs of the child or the parent/guardian of the child.
- The statement, using the form provided here within, must be acknowledged by a judge or family support magistrate, a clerk, deputy clerk of a court having a seal, a town clerk, a notary public, a justice of the peace or an attorney licensed to practice in the state of Connecticut.
- The amended statute requires the statement to be presented before a child can be enrolled in any public or non-public school program AND before a student can enter 7th (seventh) grade.
Required Health Screenings:
Vision: Kindergarten, grades 1, 3, 4 and 5
Hearing: Kindergarten, grades 1, 3, 4 and 5
Scoliosis: Females in grades 5 and 7; Males in grade 9
